Medication Algorithm Overview
Gabapentin Algorithm
Applies when CIWA-Ar is 2–7 (if appropriate and not contraindicated)
  • Day 1: Gabapentin 300 mg PO every 6 hours
  • Day 2: Gabapentin 300 mg PO every 8 hours
  • Day 3: Gabapentin 300 mg PO every 12 hours
  • Day 4: Gabapentin 300 mg PO every 24 hours
  • Day 5: Stop
Diazepam Algorithm
Applies when CIWA-Ar ≥ 8 (if appropriate and not contraindicated), with safety checks for RASS and vitals.
  • Day 1: Diazepam 10 mg PO four times daily (QID)
  • Day 2: Diazepam 10 mg PO three times daily (TID)
  • Day 3: Diazepam 10 mg PO twice daily (BID)
  • Day 4: Diazepam 10 mg PO once daily (QD)
  • Day 5: Stop
  • PRN: Diazepam 10 mg PO STAT PRN if CIWA-Ar > 8 or RASS > 2, per provider orders
  • Do NOT give any sedative medication if RASS is -1 through -5, or if SBP < 100, DBP < 60, HR < 60, or RR < 12.
